When many people hear the term pelvic floor physical therapy, they immediately think about pregnancy, postpartum recovery, or leaking with sneezing and exercise. While those are important areas of pelvic health, they are only part of the picture.
At Trailhead Physical Therapy and Wellness, we work with many individuals struggling with pelvic pain and pelvic floor dysfunction — and often, they’ve been told their symptoms are “normal,” that they just need to do Kegels, or that they simply have to live with it.
The truth is: pelvic pain is common, but it is not normal.
And effective pelvic pain treatment is about far more than squeezing muscles or doing endless core exercises.
What Is Pelvic Pain?
Pelvic pain can show up in many different ways. Some people experience constant pain, while others notice symptoms only during certain activities or stressful periods.
Common pelvic pain symptoms may include:
Pain with intercourse
Tailbone pain
Hip pain or low back pain
Pelvic pressure or heaviness
Burning, tension, or aching in the pelvic region
Pain with sitting
Pain during pregnancy or postpartum recovery
Urinary urgency or frequency
Constipation or bowel discomfort
Difficulty relaxing the pelvic floor muscles
Many people are surprised to learn that these symptoms are often connected to the pelvic floor and surrounding systems.
Pelvic Floor Therapy Is More Than Kegels
One of the biggest misconceptions about pelvic floor therapy is that everyone needs to strengthen their pelvic floor with Kegels.
In reality, many people with pelvic pain already have pelvic floor muscles that are overactive, tense, guarded, or lacking proper coordination. In these cases, constantly squeezing the muscles can actually make symptoms worse.
At Trailhead, we take a whole-body approach to pelvic floor physical therapy. Instead of focusing only on isolated exercises, we look at how the entire system is functioning together.
Our Whole-Body Approach to Pelvic Pain Treatment
Pelvic pain is rarely caused by just one thing. That’s why treatment often needs to address multiple contributing factors at the same time.
1. Addressing Mechanical Restrictions and Tension
The pelvic floor is made up of muscles, fascia, connective tissue, and nerves that can become tight, restricted, or irritated over time.
We assess for:
Muscle tension and guarding
Scar tissue restrictions
Hip and low back mobility limitations
Breathing mechanics
Postural patterns
Core and pressure management issues
Treatment may include hands-on therapy, mobility work, stretching, and movement retraining to help reduce tension and improve overall function.
2. Improving Core System Balance
The pelvic floor does not work alone. It is part of a larger core system that includes the diaphragm, abdominal muscles, back muscles, and hips.
Sometimes people have learned to constantly grip their stomach, brace their core, or hold tension throughout the day without realizing it. This can create extra pressure and imbalance throughout the pelvis.
Rather than focusing on aggressive strengthening, we help clients improve coordination, pressure management, breathing patterns, and balance within the system so the body can function more efficiently.
3. Supporting Nervous System Regulation
This is one of the most overlooked pieces of pelvic pain.
When the nervous system becomes stuck in a heightened state of protection or stress, the body can stay tense and sensitive, even after the original injury or trigger has improved.
Stress, chronic pain, birth trauma, surgeries, medical experiences, and life demands can all influence how the nervous system responds.
That’s why pelvic pain treatment at Trailhead often includes nervous system downtraining strategies to help the body feel safer and less reactive. This may include:
Breathing strategies
Relaxation techniques
Gentle movement
Education about pain science
Stress regulation tools
Gradual return to activity
Our goal is to help clients reduce flare-ups, improve body awareness, and feel more in control of their symptoms.
Pelvic Pain Can Affect More Than Your Body
Pelvic pain can impact exercise, intimacy, sleep, work, parenting, travel, and everyday confidence. Many people quietly modify their lives around their symptoms for years before finally seeking help.
We believe pelvic health care should be compassionate, holistic, and non-judgmental. Every person’s story and experience is different, and treatment should reflect that.
